Patient Care Coordinator Assistant

Assist, coordinate, implement and review the nursing care provided to home care patients. Facilitate care by working through a team of case managers, home health aides and other health care delivery professionals in accordance with patient needs, government regulations and company policies so as to maintain excellent care for the patients.
  • Must hold a current and valid clinical license (RN, LPN, SLP, OT, COTA, LPTA, etc).
  •  At least two (2) years clinical experience with a minimum of one (1) year of home health experience.
  • Complies with acceptable professional standards and practice.
  • Demonstrates good communication and organization skills.
  • Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Must be capable of performing the job functions of this position with or without accommodations.
  • Work with management staff in planning all aspects of care coordination, including communicating priorities and job assignments.
  • Coordinate patient care by reviewing reports and reporting utilization trends or variances to the Patient Care Coordinator.
  • Assist with employee training in office processes and procedures. Coordinate new employee orientation among appropriate staff. Mentor to new field staff.
  • Respond to clinical calls and direct to the Patient Care Coordinator if appropriate.
  • Perform audits and assist with quality process
  • Perform daily follow up on labs and fax labs to the appropriate parties.
  • Facilitate communication process to DME, physicians, and clinical staff.
  • Work collaboratively with the Patient Care Coordinator to monitoring staffing trends and paperwork submission
  • Review paperwork submissions for required agency forms such as consents, EDN, HHCCN.  Ensure accuracy and communicate to appropriate staff.
  • Log, trend, and report to appropriate staff of falls, infections, and incidents.
  • Monitor for utilization using the daily visit report and assist with scheduling as needed. Covers field visits as appropriate throughout the coverage area.
  • Task management, validation and / or review of communications and routine visits.
  • Seeks direction from the appropriate Patient Care Coordinator or Agency Administrator / Branch Manager.
