Summary

JOB TITLE:  Rehab Manager - North Alabama

GENERAL PURPOSE:

Responsible for implementation, mentoring, and tracking of clinical or rehabilitation programs under the direction of Rehab Director and  in coordination with Area Director. Coordinates implementation with area management, and local branch operations the processes and documentation to improve outcomes and ensure compliance with regulatory, payer or accreditation entities. Monitors therapy or clinical programs and utilization to ensure teamwork, improve outcomes, and improve the patient experience. Performs, mentors and educates clinical activities and participates in marketing events.

QUALIFICATIONS:

  • Must have a Master’s degree in Physical Therapy, Occupational Therapy or Speech- Language Pathology.
  • Must hold a current and valid therapy license in the state employed.
  • Two (2) years’ experience in a Home Health agency as a therapist, required. Two (2) years’ experience in management level position, preferred.
  • Knowledge of corporate business management, governmental regulations and CHAP standards.
  • Demonstrates good communications and public relations skills.
  • Demonstrates autonomy, organization, teamwork, flexibility, and cooperation in performing job responsibilities.  Overnight travel may be required.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Must be capable of performing the job functions of this position with or without accommodations.

ESSENTIAL JOB FUNCTIONS:

  • Ensures daily clinical operations by implementing and tracking established therapy or clinical programs and processes. Provides consultation to clinicians with established best practices or processes to ensure appropriate team work and practices to ensure appropriate patient outcomes.
  • Ensures compliance with all state, federal, and CHAP regulatory requirements. Assures proper documentation of clincal services in compliance with state, federal, and CHAP standards.
  • Educates and assists in the implementation of improved work methods and procedures to ensure clinical services are delivered in accordance with policy. Utilizes root cause analysis with rapid cycle improvement methods and actively engaged in branch specific improvement processes.
  • Educates and assists to ensure clinical services are delivered with best practice. Actively participates in recruitment and orientation of new staff.
  • Ensures seamless home care service delivery and teamwork by providing consultation and education to branch operations and clinical team that enhances coordination of care. Participates in care team conferences.
  • Mentors staff on the improvement of assessments, planning, implementation, and evaluation of clinical care. Maintains comprehensive working knowledge of home health, clinical or therapy services and shares information with appropriate organization personnel.
  • Performs skilled visits or processes workflow as needed.
  • Adheres to all of the Company’s policies and procedures.
  • Performs other duties as assigned.
